REQUIREMENTS

 

Education: Pharm.D. degree from an accredited college of pharmacy.

 

Preferred Education: Doctorate (Professional)

 

Certification: Texas State Board of Pharmacy Pharmacist license within 90 days of job entry.

 

Preferred Certification: American Hearth Association Basic Life Support (BLS) and Board of Pharmaceutical Specialty Certification (BCPS)





 

 

Experience:
Post Graduate Year 1 (Pharmacy Practice) and Post Graduate Year 2 (in the areas of specialization) residency -OR- equivalent experience as a Clinical Pharmacist in the area of specialization and five years experience in practice area of specialization.  Successful completion of the LEADing Self Accelerate program may substitute for one year of required supervisory or management experience.


Preferred Experience:
Drug Information, Formulary Management, and/or Medication-Use Safety and Policy experience
Three years hospital pharmacy experience
Pharmacy supervisory or management experience

Pharm.D. degree.
