The mission of The University of Texas M. D. The ideal candidate is a skilled healthcare professional passionate about improving outcomes for patients with prediabetes or diabetes. They excel at assessing patient needs, delivering individualized education, and developing programs for patients and hospital staff. Collaborative by nature, they work closely with an interdisciplinary team to ensure evidence-based, patient-centered care. They are committed to quality initiatives and will obtain CDE certification within 240 days of hire.


SUMMARY

Coordinates and facilitates diabetes education and care for patients with prediabetes or diabetes and their families/caregivers. Assesses patients' need for diabetes care and/or education. Develops and implements educational programs for hospital staff. Collaborates with the interdisciplinary team, e.g. physicians, advanced practice professionals, nurses, pharmacists, etc.) to provide quality patient care and appropriate use of resources. Participates and contributes to institutional quality initiatives. Impacts patient outcomes by providing, coordinating, and supporting evidence based direct patient care across the continuum of care.


JOB SPECIFIC COMPETENCIES

1. Diabetes Education and Care (60%)  
Develops, assesses, implements and evaluates education plan for patients with diabetes
Performs diabetes-specific physical, educational and psychosocial assessments
Facilitates patients' highest level of functioning through the application of culturally relevant interventions
Delivers all aspects of the educational plan to patients
Assesses and recommends appropriate interventions for clients who experience difficulties in managing condition, including basic insulin dose adjustment. Makes medication changes or follows medication adjustment protocols, or makes necessary recommendation to primary care providers. 
Develops teaching materials that support patient learning needs
Participates as multidisciplinary team member to enhance the delivery of diabetic patient care

2. Team Collaboration in Diabetes Education Services (25%)  
Teaches classes about diabetes management basics and sick day care for caregivers and nursing staff
Glycemic surveillance: Reviews daily reports on inpatients that have significant glycemic abnormalities and works with inpatient teams to see what insulin adjustments are needed or if endocrine consultation is needed
Assists with discharge process for diabetic patients.
Creates educational materials for patients undergoing surgical procedures and/or imaging, including patients who use insulin pumps
Creates educational modules for providers and other members of institutional staff about management of glycemic abnormalities, including information about glucose monitoring, basal/bolus insulin therapy and appropriate dosing
Collaborates with Patient Education Department to develop and monitor educational resources to meet the needs of the diabetic population


3.Strategic Program Development (10%)   
Functions as a multidisciplinary team member, working with physicians to enhance the delivery of diabetic patient care
Participates in educational and clinical research to provide optimal services to patient with diabetes
Trains, coordinates, and develops staff to assist in the delivery of diabetic teaching to patients and families
Assist in pursuing ADA certification for diabetes self-management program, if determined feasible by department or institutional management

EDUCATION: Bachelor's degree or graduation from an accredited school of professional nursing or dietetics. 

EXPERIENCE: Five years clinical experience as a nurse or dietitian. Must have a Certification in Diabetes Education (CDE) within 240 days of entry into the job and Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification. 

LICENSURE: Must also have one of the following: Current State of Texas Professional Nursing license (RN); Current Registered Dietitian (RD) with the Commission on Dietetic Registration (CDR) and Current Licensed Dietitian (LD) with Texas State Board of Examiners of Dietitians. Must also have one of the following: Current State of Texas Professional Nursing license (RN) Current Registered Dietitian (RD) with the Commission on Dietetic Registration (CDR) and Current Licensed Dietitian (LD) with Texas State Board of Examiners of Dietitians.

This position may be responsible for maintaining the security and integrity of critical infrastructure, as defined in Section 113.001(2) of the Texas Business and Commerce Code and therefore may require routine reviews and screening. The ability to satisfy and maintain all requirements necessary to ensure the continued security and integrity of such infrastructure is a condition of hire and continued employment.
