The Social Work department is here to help patients, their families, and their caregivers cope with the diagnosis of cancer and to eliminate psychosocial complications as significant barriers to care in Texas, the nation and the world through outstanding integrated programs in patient care, research, education and prevention.

The ideal candidate will only work 19 hours per week, 2 days during the week, and not including weekends. This is a per diem role.

Summary

Summary

The primary purpose of the social work associate position is to coordinate care, provide resource education and linkage and provide education about advance directives. The social work associate will triage patients needing additional assessment and a higher level of care to social work counselors. The social work associate will also collaborate and coordinate with the social work counselor to ensure care needs are met. Additional duties may involve reviewing completed advance directives for accuracy and identifying community resources to increase access to care. The social work associate may also act as a liaison between the department of social work and community resources as well as complete other duties as assigned.

Key Functions

1. Coordinate care meetings related to goals of care and advance care planning amongst the patient, family and medical team members. Provide education of advance directives and facilitate completion of advance directives as needed. Review completed advance directives for accuracy and release in electronic health record. If document is inaccurate, follow-up with patient regarding inaccuracies to provide education on the role of advance directives.

2. Provide resource information and linkage to patients and families as needed, identify community resources and act as a liaison to community agencies. 

3. Coordinates and collaborates with social work counselor and health care team to reduce barriers to care.

QUALIFICATIONS  

Education

Required:  Bachelor's degree in Social Work (BSW) from an accredited program approved by the Council on Social Work Education (CSWE).

Experience

Required:  None.

Licensure

Preferred:  Licensed Baccalaureate Social Worker (LBSW) from the state of Texas.
