Transitional Care Manager
EDUCATION
Required: Bachelor of Science in Nursing from an accredited school of professional nursing.
Accreditation Oncology Nursing Certification (ONCC) and/or Case Management Certification
EXPERIENCE
Three years oncology experience in hospital or clinic setting or three years of experience in Care Management. Care Management experience strongly preferred. Experience speaking with patients and family at home on the phone. General work experience, including use of computers for data entry, research and producing written reports. Must be proficient in Microsoft Word and use of Internet research skills.
Preferred: Telephone help line, counseling, teaching or customer service role in oncology or population health. Bilingual Spanish/English
LICENSE/CERTIFICATION
Active RN license in the State of Texas or eligible to obtain within 90 days of hire
