EDUCATION:
Required: Bachelor’s degree in a health related field.
EXPERIENCE:
Required: Five years of related program management experience. Two years of experience in implementation and support for initiation of a transitions in care phone program for an inpatient multi-service hospital system and EHR (EPIC) implementation. May substitute required education degree with additional years of equivalent experience or completed years of college on a one to one basis. With a Master’s degree, three years of required experience.
Preferred: Experience in marketing for transitions in care programs internal and external.
